#110 Champix - Varénicline

Ce médicament commercialisé par Pfizer a la particularité  de se fixer spécifiquement sur les récepteurs à la nicotine qui se trouvent dans le cerveau. Comme c'est la nicotine qui est responsable de la dépendance, ce nouveau médicament inhibe la dépendance. 

Selon e-sante.fr,  "le mécanisme d'actions est double :
il stimule la libération d'un neurotransmetteur cérébral (dopamine), ce qui atténue le besoin impérieux de fumer et les symptômes de sevrage (irritabilité, troubles du sommeil, dépression, anxiété…) ;
il empêche la nicotine de se fixer sur les récepteurs, ce qui atténue la satisfaction associée au tabagisme et diminue ainsi le risque de rechute."

Effet "indésirable" potentiel? Des idées suicidaires :(

                      Point d’information sur Champix ® (varénicline)

                                                                                                                               

L'Emea et l’ensemble des Agences européennes souhaitent alerter les professionnels de santé et les patients sur le risque d'idées suicidaires ou de tentatives de suicide survenant lors des traitements avec Champix ® (varénicline) à la suite de l’observation de ces évènements. Bien que la relation entre la prise de Champix ® et la survenue de ces complications ne soit pas établie, il est apparu nécessaire d’informer sur ce possible risque en raison des conséquences possibles. Au moment de la commercialisation de ce médicament qui est indiqué dans le sevrage tabagique, l'EMEA a mis en place un plan de gestion des risques afin de suivre les effets indésirables du médicament, qui font aussi l’objet d’un suivi national de pharmacovigilance.

                                                       

Champix ® (varénicline) est un médicament indiqué dans le sevrage tabagique chez l'adulte. Il a obtenu une autorisation de mise sur le marché européenne septembre 2006. Il est commercialisé par les laboratoires Pfizer en France depuis février 2007. Sa commercialisation a été accompagnée d'un plan de gestion des risques européen et national.

               

L'Emea a analysé les données européennes de pharmacovigilance depuis son autorisation de mise sur le marché. Des cas de troubles de l’humeur et des idées suicidaires voire exceptionnellement des tentatives de suicide ont été analysés à plusieurs reprises, en juillet, octobre et novembre 2007.

               

En conséquence, l'Emea a conclu lors de sa réunion de décembre 2007 qu'il était nécessaire d’actualiser les informations destinées aux professionnels de santé (le Résumé des Caractéristiques du Produit) et aux patients (notice) pour les mettre en garde sur le risque de dépression, d’idées suicidaires ou de tentative de suicide survenant chez les personnes souhaitant arrêter de fumer avec Champix ®.

               

La relation de causalité n’est pas établie entre la prise du médicament et ces symptômes, qui peuvent apparaître également lors de tout sevrage tabagique sans médicament. Toutefois, l'Emea a demandé au laboratoire Pfizer de proposer des modifications pour le résumé des caractéristiques du produit et de la notice patient avant le 19 décembre 2007.

               

L’Afssaps rappelle que tout effet indésirable grave ou inattendu susceptible d’être lié à la prise de Champix ® doit être obligatoirement déclaré par les professionnels de santé aux centres régionaux de pharmacovigilance.
