The Bloomberg Initiative received $375 million from Michael Bloomberg (in 2006 and 2008) and an additional $30 million from the Gates Foundation (as of July 2009). That's a total of (about) $405 million from 2007 to 2011 (5 years). That's about $81 million per year, divided between 5 entities (CTFK, The Union, The World Lung Foundation, the CDC Foundation and The Johns Hopkins School of Public Health).
The new 4 year grant of $220 million represent $55 million per year (without -for now- any additional funding from the Gates Foundation since the $18 million grant that had been mentioned in July 2011 has not been confirmed)
For SubSahara Africa it is only possible to track the grants awarded by CTFK, at least if they are all made public. CTFK is the only organization to do so. It is possible that the Union supported financially certain projects and CDC did fund a few prevalence surveys but (as far as I know) no financial details have been made public.
As far as CTFK is concerned, the total of the grants they awarded to projects in SubSahara Africa from 2007 to January 2012 is $4.405.184 ($881.036 per year for 5 years, including the most recent grant to Ghana in January 2012). That does not include many meetings, travel and hospitality etc... that are not publicly available but that gives a guestimate.
As for the Gates Foundation, they gave $5.2 million to IDRC in december 2007, $7.884.847 to ACS for the Consortium ($ 7 million in November 2009, increased in 2011),$9.994.493 to WHO on July 31 2009 (for the African Hub), $451.720 to ACS for the Joburg conference (october 2010) for a total of about $23 million+ ($4.6 million per year for 5 years)
Bloomberg + Gates = (at least) $27 million
